Public Service form three PSF3, (Revised 2008) should be filled in triplicate with 3 recent coloured Passport Photos, Certified Copies of Academic Documents, Registration Certificates, Practicing Licenses, Academic and National Ids MUST be attached and submitted to the Secretary, Lyantonde District Service Commission, P.
